How It Works

Purchase the plan

Your official membership kit will arrive in the mail within 7 to 10 business days.

Locate a participating provider

Visit the Provider Search pages for your plan or by call (844) 442-5622, any time between 7 a.m. and 7 p.m. CT, and we can help find a provider in the network that accepts your plan.

Present your card

At the time of service, show your member ID card and receive a discount off the regular-priced fee. It's that easy.
